前庭周围性眩晕是临床最常见的眩晕类型,具有高发病率、反复发作等特点,严重影响患者的日常生 活。前庭周围性眩晕疗效测评量表是前庭周围性眩晕临床诊疗与研究的重要工具,眩晕障碍量表(DHI)、平衡 信心量表(ABC)、Berg 平衡量表(BBS)等常用疗效测评工具普遍表现出较好的信效度[如 DHI 中文版克隆巴赫 系数(Cronbach's α)为 0.751~0.918,ABC 中文版 Cronbach's α 为 0.95],但前庭眼动筛查(VOMS)等量表的信 效度研究仍较缺失。在最小临床重要差值(MCID)方面,DHI、ABC 及 BBS 量表的相关研究较多,但结果存在 较大差异,而眩晕症状量表(VSS)、前庭疾病日常生活量表(VADL)、前庭康复获益问卷(VRBQ)等量表尚未建 立明确的 MCID 阈值。现有核心量表在信效度层面总体可靠,但 MCID 结果存在缺失,部分量表关键指标证据 不足,需进一步完善。该文系统梳理了前庭周围性眩晕常用临床疗效测评工具的信度、效度及 MCID,分析前 庭周围性眩晕结局评价工具的研究现状,以期为前庭周围性眩晕的疗效评价工具及临床诊疗决策优化提供科学 依据。

Peripheral vestibular vertigo,the most common type of vertigo in clinical practice,is characterized by high incidence and recurrent episodes,significantly impacting patients’ daily lives. Efficacy assessment scales for peripheral vestibular vertigo are crucial tools in its clinical diagnosis, treatment, and research. Commonly used efficacy assessment instruments, such as the Dizziness Handicap Inventory (DHI), Activities-specific Balance Confidence (ABC) scale,and Berg Balance Scale (BBS),generally demonstrate good reliability and validity (e.g.,Cronbach’s α for the Chinese version of DHI ranges from 0.751 to 0.918,and for the Chinese version of ABC,it is 0.95). However, research on the reliability and validity of scales like the Vestibular/Ocular Motor Screening (VOMS) remains insufficient. Regarding the Minimal Clinically Important Difference (MCID),while numerous studies exist for the DHI,ABC,and BBS,their results show considerable variability. Furthermore,specific MCID thresholds have not yet been established for scales such as the Vertigo Symptom Scale (VSS),Vestibular Activities of Daily Living (VADL),and Vestibular Rehabilitation Benefits Questionnaire (VRBQ). Although the core existing scales are generally reliable in terms of their psychometric properties, there is a notable lack of established MCID values, and evidence for key indicators in some scales is insufficient, necessitating further improvement. This paper systematically reviews the reliability,validity,and MCID of commonly used clinical efficacy assessment tools for peripheral vestibular vertigo, analyzing the current research status of outcome evaluation instruments for this condition. The aim is to provide a scientific basis for optimizing the selection of efficacy assessment tools and informing clinical diagnosis and treatment decisions for peripheral vestibular vertigo.
