除湿胃苓汤出自清代吴谦的《医宗金鉴》,是健脾燥湿、和中利水的经典名方。现代研究表明,该方在 湿疹、带状疱疹、皮炎等疾病的治疗中效果显著,具有调控促炎因子水平、维持 Th1/Th2 细胞因子的稳定、调 节氨基酸及其衍生物代谢等药理作用。本文系统梳理了除湿胃苓汤在化学成分、药理作用及临床运用方面的最 新研究进展,并基于质量标志物(Q-Marker)“五原则”的理论框架,对其进行质量标志物预测分析,结果表明 苍术素、苍术酮、绿原酸、厚朴酚、和厚朴酚、橙皮苷、川陈皮素、麦角甾醇、23-乙酰泽泻醇 A、23-乙酰泽 泻醇 B、泽泻醇 A、泽泻醇 B、去氢土莫酸、白术内酯Ⅰ、白术内酯Ⅱ、白术内酯Ⅲ、升麻素苷、5-O-甲基维 斯阿米醇苷、栀子苷、藏红花酸、桂皮醛、木通苯乙醇苷 B、厄弗酚、灯心草酚、甘草苷、甘草酸、甘草次 酸、含水硅酸镁为除湿胃苓汤的质量标志物,可为经典名方除湿胃苓汤的药效物质基础及质量评价标准提供参 考依据。

Chushi Weiling Decoction, originating from Wu Qian’s Yizong Jinjian(Golden Mirror of the Medical Tradition) of the Qing Dynasty, is a classic formula renowned for strengthening the spleen to dry dampness, harmonizing the center to promote diuresis. Modern research indicates that this formula demonstrates significant efficacy in treating conditions such as eczema,herpes zoster,and dermatitis. Its pharmacological effects include regulating the levels of pro-inflammatory factors,maintaining the balance of Th1/Th2 cytokines,and modulating the metabolism of amino acids and their derivatives. This article systematically reviews the latest research progress regarding the chemical constituents,pharmacological effects,and clinical applications of Chushi Weiling Decoction. Furthermore,based on the theoretical framework of the “Five Principles” for quality markers (Q-Marker),a predictive analysis of its quality markers was conducted. The results suggest that atractylodin, atractylon, chlorogenic acid, magnolol, honokiol, hesperidin, nobiletin, ergosterol, 23-acetyl alisol A, 23-acetyl alisol B, alisol A, alisol B, dehydrotumulosic acid, atractylenolide Ⅰ , atractylenolide Ⅱ , atractylenolide Ⅲ , cimicifugoside, 5-O-methylvisammioside, geniposide, crocetin, cinnamaldehyde, calceolarioside B, effusol, juncunol, liquiritin, glycyrrhizic acid, glycyrrhetinic acid,and hydrated magnesium silicate are potential quality markers for Chushi Weiling Decoction. This analysis aims to provide a reference for understanding the pharmacodynamic material basis and establishing quality evaluation standards for this classic formula.
